What the manufacturer publishes
The Lilly Zepbound coverage and savings page lists regular self-pay prices of $299 for 2.5 mg, $399 for 5 mg, $499 for 7.5 mg, and $699 for 10, 12.5 and 15 mg. A $449 Self Pay Journey offer may apply to 7.5–15 mg if the refill is purchased within 45 days. This is a conditional program price, not a permanent list price.
Compare a specific fill
If your confirmed 5 mg cash program price is $399 and your plan quotes $280, enter both routes. The worksheet shows $280 for one fill and $3,360 across 12 identical fills. It cannot know whether future strengths, insurance phases or program rules will change.
Ask the pharmacy whether its quote is for a vial or KwikPen and includes any fees. Manufacturer copay and self-pay terms differ, especially with government coverage. Do not use a lower price without checking eligibility.
